The Center for Pain Medicine provides a comprehensive approach with multiple treatment options to treat individuals who suffer from chronic pain associated with an injury or illness.

Please call the scheduling line for the clinic in your area to start the process of becoming a patient.  Be sure to notify the scheduler if you have a current referral from a physician.  If you do not have a physician referral you will be asked a couple of brief questions, which the physicians will review in order to make sure that you are a candidate for our pain medicine practice.  For those without a physician referral, our pain physicians will make the determination of whether or not to accept you as a full time patient only after the initial consultation.

If you are unable to keep your appointment, please give us as much notice as possible. By calling 48 hours in advance, you give us the opportunity to see patients who are requesting an earlier appointment.

Financial Arrangements

Please bring sufficient insurance information with you. Depending upon the type of insurance and your policy, hospital benefits can vary. We will do our best to ensure that you get all the benefits that your policy allows. Here is a list of our insurance providers:

We will bill your insurance on your behalf as a courtesy to you. Although your insurance may pay a portion of the bill, it is your responsibility to pay for your deductible and your co-payment amount.

For patients without insurance, payment of the estimated bill is due prior to time of service. For further questions, please call Florida Hospital's Patient Financial Services at 407/895-7774.

You should expect to receive a separate bill for the Pain Medicine physician's professional services provided. For further questions, please call 407/667-0505.

Instructions for Day of Procedure

  • You must arrange in advance for a responsible adult to remain available during the procedure and drive you home following the procedure (if you receive sedation for procedure you will not be permitted to drive yourself home).
  • Please arrive 10-15 minutes prior to your scheduled appointment.
  • Do not eat or drink anything (including water, candy or chewing gum) 8 hours prior to your scheduled appointment unless otherwise directed by your pain physician. You may take scheduled medications with a few sips of water.
  • Leave all jewelry and valuables at home.
  • Wear loose, comfortable clothing (low-heeled shoes for women).
  • Bring a list of all your prescriptions and over-the-counter medication (with Dosages)
  • Bring your insurance card / information
  • Bring in any X-rays, MRI's, etc. that may be pertinent to your pain management

Telephone Calls

When you call to report pain concerns, a message will be taken by the receptionist. You will be called back at the end of the day or when the staff is not busy with other patients. If you feel your symptoms warrant a doctor's immediate attention, go to the Emergency Room closest to you.

Prescription Refills

Medication refill requests must be submitted during regular office working hours only. Refills must be called in 24 hours in advance to allow time to process your prescription. Refills called in on a Friday will be available Monday afternoon.

Certain medications may require office visits in order to obtain refills while other medications can be called in to your pharmacy.
